At Compass Health Network, our Health Home programs are designed to support the whole person—bringing together behavioral health, primary care, and developmental disability services into one connected, compassionate approach. We understand that each individual’s health journey is unique, and our goal is to ensure every person has access to coordinated, personalized care that meets their needs, preferences, and goals.

Health Home is a free Medicaid service that surrounds you with a dedicated care team focused on your overall well-being. This team may include a nurse care manager, behavioral health specialists, care coordinators, and other specialized healthcare providers who work together—and with you—to plan, communicate, and coordinate your care. By connecting services across providers and settings, we help reduce barriers, improve health outcomes, and make navigating healthcare simpler and more supportive.

Whether you are managing a chronic condition, working toward improved mental wellness, or seeking additional support for developmental needs, Compass Health Home programs are here to help you feel supported, informed, and empowered—every step of the way.

Who is eligible for Home Health Services?

Compass Health Home programs are available to individuals who are currently receiving Medicaid (MO HealthNet) benefits and have qualifying health conditions. Health Home services are specifically designed to support individuals with ongoing or complex healthcare needs through coordinated, whole-person care.

Compass Health Network is proud to offer all three Missouri Medicaid Health Home programs:

  • Behavioral Health Home
  • Primary Care Health Home
  • Developmental Disabilities Health Home

These programs are supported and managed by the Missouri Department of Social Services (DSS) and the Department of Mental Health (DMH), with eligibility guidelines that vary based on the program.

In general, Health Home services may be a good fit if you:

  • Have a mental health condition or substance use disorder
  • Are living with one or more chronic health conditions (such as asthma, diabetes, heart disease, or similar ongoing health concerns)
  • Have an intellectual or developmental disability and may benefit from additional health coordination and support
  • Could benefit from help managing your health, navigating services, and connecting your care across multiple providers

What a Health Home Is Not

To help clarify, a Health Home is not a physical place or a change to your existing care—it’s an added layer of support designed around you.

  • Not a place you live – A Health Home is not a house or residential program, and it will not change where you live or your current living situation.
  • Not a replacement for your doctors – You will keep your primary care provider and any specialists you already see. Your Health Home team simply adds another layer of support to help coordinate your care
  • Not a change to your Medicaid benefits – Health Home services are an added benefit and do not take away, replace, or reduce the services you currently receive through Medicaid

Health Benefits of Health Home Team
